what might be the consequences for a patient who is taking a drug with a narrowm therapeutic range if their GFR is lowered?

A low GFR would mean less filtering of any substance from the body, including drugs, so that would probably increase the drug's action/effect, because the drug is in circulation longer.

they would have a higher blood pressure due to a higher volume of blood in the vessels, which is due to the lower GFR.
