Well why don't you get him something that the two of you might have shared especially when you were very young.Something significant to both of you.So perhaps it may brings back memories of the good and happy time you shared with him.Moreover it will help to remind him of you,the wonderful and loving sister who cares very much for him.That would be great,won't you think so?

how about a day out - cinema (his choice of film), (take away - his favourite), or you cook and make a cake, pub in the evening for a few drinks? If this is too expensive, favourite aftershave or skin care stuff, but nicely wrapped?
