Heart attack is less common for females till menopause compared to males due to hormones. After menopause, both males and females are equally affected by heart attacks.

It is possible. There can be undetected causes or even a congenital condition that has gone untreated. Usually being overweight and having higher than normal cholesterol and triglycerides go hand in hand. When was his last checkup with blood tests? When was his last checkup with a cardiologist? Also what prompted his doctor to have him go through a cardiac stress test in the first place? He is doing most of the right things for a healthy heart, except for the weight. If his cholesterol and triglycerides were high in the recent past it could have caused narrowing of coronary arteries, thus the chest pain. There is also the possibility of problems with the electrical conduction pathways of the heart. Most of your questions are best asked of his doctor/cardiologist.

You need to define heart disease before you can get a good answer to your first question. A person with a heart valve malformation, could be said to have heart disease that is not dependent on ones cholesterol or triglyceride level.

A person's sex appears to have an effect on heart disease. Men suffer from heart disease at a higher incidence than pre-menopausal women, but after menopause, the reverse is true.

With age plaques build up in major vessels and a person can have a heart attack. Women did not used to have them as often as men, but now it is even. Drugs like Lipitor have been recommended, but I take an 81 mg aspirin daily.
