I don't know where you live?? Is she checking her blood sugar several times a day? Is she on medication for her BP? Who prescribed her medication and who does she see for her diabetes? Because THAT is who she needs to see. Now. Forget "economical" treatments. You need to get her to a doctor right away. Just pay for it. She IS your grandmother.

Self medication and treatment is dangerous, because little knowledge is dangerous. There are several things to consider before a correct dosage is given. All substances are poisons unless they are taken with the correct dosage based on the age, weight and metabolism of the person. So don't take chances. Consult your doctor.

for diabetes - metformin and possible once daily long acting insulin

for bp problem - a calcium channel blocker such as amlodipine and a thiazide diuretic called indapamide.

of course we would need to know about her renal function and whether or not she had any problems with gout before a prescription was done.
