Anything less than 15% means you were very unhappy with the service. 20% is average.
It's not just about respect. In most states, "tipped employees" are excepted from all or part of the minimum wage laws. That means the employers can, and most do, reduce food server wages by the expected tips they should be receiving, so without normal tips they are not earning enough to live on.

15-22% is standard. 15% is what's expected, and if you have good service you should leave more, up to 22%. I don't really think you could leave less than 10% without being rude. Even that will be somewhat upsetting to a waiter, but if you feel you actually had BAD service, and it was the fault of the waiter, then there are always situations that cause for different circumstances.

15% is the standard, 20% is normal for exceptional service. I think that people today over tip for the service that they receive. If you have a problem with your dining experience, speak to a MANAGER and have them discount or comp some menu items.... Do not ever 'stiff' the waiter/waitress as they are not paid much per hour and depend on tips.... The exception being if they are totally indifferent to you even being there. Hope this helps.

That depends on the place, but I would say that it's the 15% of the whole bill

Look to see what the tax is on the meal - 10% - and that is what you could leave.
