Nicotine restricts blood vessels.
Tar interferes with O2-CO2 transfer in the lungs.
Carbon Monoxide attaches to the red blood cells and prevents them from carrying O2.
Many other factors are occurring but the end result is much less O2 reaching cells.

I'm assuming you mean from Cigarettes. These are just off the top of my head, there's probably more: -
LUNGS: - Emphyxema, Persistent hacking coughs, lung cancers, damage to the lining of the lung, Chest Infections, decreased oxygen absorbtion.
BLOOD: - Decreased level of oxygen in blood, easy transmittance of cancer causing radicals to other parts of the body, less white-blood cell production, collapsed capillaries.
BRAIN:- Addiction, less oxygen reaching the brain, brain cancers, bad memory, memory loss, other neural defects.
CANCERS: - Lung Cancer, Throat Cancer, Mouth Cancer, Cancer of the Nasal Passage, Brain Cancer, Skin Cancer, Liver Cancer, and a higher risk of contracting any other form of cancer on the body.
