I have seen an attachment that goes onto the pram, it is just a small platform on wheels which goes between you and the pram, you might be able to make one.

Well, there's the Sit'n'Stand.

And there's the Joovy Caboose.

And Litaf makes an attachment called the Hitchhiker that fits almost all strollers.

But you need to try them with your toddler before buying one. We were going to buy a Joovy, but our then two-year-old hated it. And when you use a car seat in the front for the infant, it really takes away from room for your toddler. Plus, it's really heavy and hard to steer.

I don't know about the Sit'n'Stand.

We had a Hitchhiker. It worked OK for us, but we do a lot of city walking, and it wasn't so great bumping over tons of curbs. And we live in Minnesota, and it wasn't so hot in the snow, either. Plus you had to attach it or disattatch it whenever you wanted to fold or set up the stroller.

We ended up buying a used Phil & Ted's E3.

The first time that I saw this stroller, I had to have it too! My daughter is 4 and my son is 1 1/2. She always wants to get out and walk. Our double stroller is just too large to lug around. At One Step Ahead, they have it. It looks like good quality. I have also seen them on ebay, at babies r us and now at Target. The one at Target is the cheapest; maybe lower quality? Most of them are around $150.

I have seen them but not sure where you can get them. If I was you I would try Toys R US or Baby Depot. Good luck looking cause I believe they cost a pretty penny too.
