This form is a questionnaire in which a court wants personal info such as name, address, date of birth, etc. to be used in considering a person as a potential juror. The "subpoena" says that if the person doesn't respond they may be subject to a fine and/or prison. The reason the form says "Juror Subpoena Failure to Respond" is because previous questionnaires were not answered.

Subpoenas can be either hand delivered or mailed, however, they are usually sent certified mail to make sure the recipient actually received it. Check with the court house that mailed the subpoena to make sure it's legit if you have any questions or concerns about it.

Definitely check out the legitimacy of the questionaire, if it really is for you, then it would come from the county where you live and nowhere else. It could be a "phishing" scam like other people here have told you.....don't give out any info at all until you speak to the jury office of the county where you live, it sounds very suspicious!

It sounds legitimate, but if you are suspicous call the County Court house and verify if there are any pending jury trials that prompted the mailing.

The Courts already know everything about you. Why would they need a questionnaire?

I wouldn't answer it, sounds like it could possibly be a scam. You need to call the courthouse to find out if it is legit or not.
