Many things. You said the dough comes out heavy. That means you put too much flour in it. Make sure when you measure the flour that it is fluffy and not packed at all. To make it rise turn the oven onto warm and then turn it off. Place the dough in the oven to rise for 1-2 hours and then if you want knead again and put it in the oven for another hour. One other thing that could help a lot is to buy self rising flour in addition to using your yeast. The self rising flour will help it rise big time. Good luck

If you know your yeast is OK, and the water temp is OK, then try this. While you're making it, turn on the oven to warm. Then shut it off and put the dough in under a clean dishtowel. Just a really mildly warm oven. That keeps it out of a draft.

Make sure the room where the dough is to rise is warm enough too; If it's too cool the yeast won't activate.

Once it's in the dough form, let it sit for at least an hour out in the open, trust me it will rise.

You may be overworking the dough, should only have to knead it for a minute or two, then let it rest. cover it with a linen or cheesecloth towel and let it set at room temperature and it should rise.

Either the yeast has died (check experation date) or you used the wrong water temp. (too hot) and killed the yeast.
