The 12 tribes existed together for quite a time, and I am not sure that God has listed all of the leaders. Moses was the main leader, but then he had leaders over every tribe, plus sub leaders.. After Moses died the 12 tribes still went into the promised land and they would have had different leaders ...

the sons of isreal (jacob) joseph is included in some places, but does not have a tribe. the levites were the priests. read genesis 49. it pretty much spells it out. (it's in the link)

When? Over time they aged and changed and we have several times when people are named, often without attributation to the tribe, or at least to all the tribes.
BTW, it is ISRAEL--you can remember the AE spelling in that EL is the generic name of God, IsraEL.
