Leslie: You don't file for disability with the Internal Revenue Service - you have to do it through Social Security. Call 800-772-1213 and ask for the Social Security Disability forms. Then you need your doctor(s) to give them proof that you are permanently disabled and unable to work. It's a long process, but wrth it if you are not going to be able to work again. If your disability is temporary, contact the EDD and file for disability - again, you will need a letter from your doctor(s).

The first step to filing an SSD (social security/disability) app is to call 1-800-722-1213!! They'll set up the phone interview with your local office.
Good luck!

You don't file for disability with the IRS, you file for disability with the Social Security Administration. I have heard it's difficult to get disability approved, and if denied you should get an attorney if you need to try again.
