First they would have to take you to court and get a judgment against you in a court. If they do that and you still don't pay, then they can go to court to garnish your wages but they must do it in that order. I seriously doubt a medical bill will go that far. The most I imagine they'll do is send the debt collectors after you.

no one can garnish your wages without your written permission or a court order for a lawful purpose except the irs or state child support agency.
