The initial drug tests are done with you in your street clothes or physical training (PT, ie gym) clothes. There will be someone watching, probably NOT a doctor, but instead a lab tech, medical assistant, or an NCO (sergeant). Depending on the type of toilet at your MEPS center, he will either be behind you or if it is a trough type toilet, he may be on the side. Usually several guys will be going (or trying to go) at once, so it's not like he will be staring at you, but he probably will see something, at least your behind.

technically someone has to watch the urine exit from your body and into the container, you must not touch the top of the container as it might contaminate the sample, you must close the container in the process and hold the container in such a way that it is visible to the witness of your actions. than the lab tech will place an appropriate seal around your sample and you must sign that seal, sign the roster and ensure that the seal, with your bottle, and the roster information is accurate and correct. than you can wash your hands.
i have monitored my share of piss tests in my day, my buddies were the piss test NCOs, than officers later on, nothing like seeing your new battalion commander, than your command sergeant major pee into a cup, bad horrifying images
think of it this way, at least all your buddies are not telling you to hurry up and pee so everyone can be released, or you might get put through force hydration or getting smoked until you can.
i heard underwear is required now for MEPS, too many guys going commando already i guess

No, you can urinate in semi privacy for the initial sample (toilet door open).
If you end up picked for compulsory Drug Testing (everyone does at some point in their career, I have twice in 6 years), you are watched by a CPL dubbed the "Wee Watcher". He just stands behind you while you urinate.

I'm not sure about for the army, personally i would hope they would be very caustious but not to that extreme. When taking a urinary test for a job I was just always asked to empty my pokets and then patted down and when in the restroom

Yes, someone will watch you pee into that cup. That validates the test that you are not using someone else's urine to pass a drug screen. With women, usually there is just another woman outside the stall, but you have to empty your pockets and get the pat-down to ensure you are not carrying urine. But for the men, yes, they are watched. Not stared at, but you know...the glances.

Not unless you pee through your finger. It is funny you should ask this because I gave a sample just last week. I am Male and I peed in a sample bottle in the patients toilet. If your Doctor wants you to do your peeing in the nude REPORT HIM/HER they are into cheap thrills. Either that or they are into playing odball power games. But personally I think that they should show respecvt for your privacy.

The last I knew someone stands there watching you go to make sure that you are really peeing and not putting in some fake stuff.

I can say from what all my buddies have said men do have someone right there to watch. Not staring but quick glances. Dont worry about it dude. You'll have to group shower during basic so pee test are teh least of your worries.

i just got back from MEPS today, i passed.
but to answer your question, no, you don't stand there naked but there will be someone standing behind you to make sure you aren't doing anything you shouldn't be, i think its the same for all branches, i didn't join the army myself, but it probably wont be any different for you. hope i helped.

HAHA its not that bad. Yes there will be someone with you to make sure you don't add any foreign substances into your sample but its not like you are naked under a microscope.
