I often get tight muscles/knots in my shoulders and neck. Anyone have any recommendations? Massage/physical therapy do little, it seems.

This is a problem in the medical / health field that bothers me. If a type of treatment doesn't work, you should not continuously get that treatment. As a physical therapist, when a patient is not showing improvement subjectively or objectively I always reassess and change the plan of treatment.

Have you considered the possible causes of your muscle tightness? Poor posture, poor ergonomics at work or home, stress related, activity driven, position with ADLs and sleeping. Maybe it is being referred from a problem in your neck. You need to find out the cause of the muscle spasms to stop them from reoccurring. Heat and Massage could help on a temporary basis.

Depending on how bad the pain is, you can seek out a chronic pain center in your area and make an appointment to see a physiatrist. They may be able to help beyond the massage / PT.

Maybe the massage you're having is not deep enough.
Deep tissue massage works with me.

I sit at a machine all day for my work, then the computer for a good part of the nigh.
It's hell on neck and shoulders.

I treat myself to the 1/2 hr "Executive Massage" monthly and it loosens those muscles well.

I'm a massage therapist and specialize in neck/shoulder issues. I suggest finding another therapist... Good one's are tough to come across, but keep searching. It really helps with the rite hands touching you.

You must be stressed out. I recommend that you use a heating pad for 15 minutes. Then put some ben-gay on your muscles that hurt and maybe change where your sleeping.
