He definately should at least call his doctor,a hospital emergency room, or a public clinic, like Planned Parenthood. I'm not sure if
it can do permanent damage, but I believe the directions say to
call a health proffessional if it lasts more than 4 hours! I'm guessing that he took much more than the recommended dosage.

i thought you should seek medacail advice {see a doc} after a 4 hour hard one .... i'd say yeah give the doc a call it could mess him up so it wont work again
