I am using chapsticks as my wedding favors, does anyone know how to make personalized lables for chapsick?

buy sticker computer paper. Choose a font, a color, and what you want it to say. Be sure the message will fit on the chapstick. Instead of the pre-cut labels I would use uncut paper so you can cut them as small as needed with scissors.
It's fun and easy. I also like to use Printshop Deluxe. With it you can add all kinds of graphics, or your initials.

Just buy stick on envelope labels as they will stick on easily and are easy to print on.

If you can't find envelope labels, just print on normal paper, cut it to the right size and then use double sided tape to stick it onto the chap stick. I can't say enought good things about double sided tape. We discovered it for my sister's wedding and I was just annoyed that I had not discovered it for my own wedding. We used it for everything eg sticking translucent paper to card (couldn't even see tape even though you could see through the paper!) and sticking ribbon to candles (it stuck on really well to the candles despite the fact that there were quite slippery).

I hope you're going to do something more fun than just putting your name and date on the chapstick. It would be great to use normal chapstick as a template and make it a funny version of that.

Quite simply use an address label template. You can buy silver and gold address labels and print them out your self. Write something like

Thank You for Joining us on our special day.
John and Jane January 1st, 2007

Put them longways and no one will be the wiser. Congrats and good luck.
