Signs are usually all over the place warning you that a toll road is ahead. So, it seems that being on one accidentally is nearly impossible. However, the attendant can have you pulled over and the police will deal with you. I am sure the way you handle the situation would determine the way you would be treated.

I am only speaking for the toll roads in Illinois. There are signs that tell you there is a toll coming up well in advance. Video cameras are at all toll road plazas.
There are three types of tolls you can go through in Illinois:
1) Open-road tolls. This means there are no individual toll booths. Drivers don't stop, they keep driving at their normal speeds. The tolls are collected electronically from steel beams overhead. Only people with an IPASS transmitter in their windshield can use these tolls.
2) Manual toll booth. An attendant is on duty. You should have enough money to pay them whatever the toll is, as the tolls are usually $1.00 or less, the exception is the toll plaza by the IL/WI border which is $1.50. If you don't have the money, then I'm sure they will have no problem calling the police to nail you right there for a toll violation. At least one state police car is usually parked by the toll plaza.
3) Automatic toll booth. There's no attendant on duty. If you go through one of these without paying, a camera will capture your vehicle and license plate and they will send you a violationin the mail.
If you happen to be at a manual or automatic toll booth, you can take a chance and ask the person behind you if they can give you some money. I can't guarantee they will help you out though. Basically if you go through any of these without paying, there's a high probability they will take a picture of your car and plate and send you a nice letter stating you owe them money. It's happened to my brother a couple of times.

Toll Roads are usually really well signed a fair way in advance so you still have time to turn around before you get too close to the tolls. It's happened to me more than once, while driving around in Europe.
I suppose if i got stuck though, I'd wait till another car drove up behind me and ask them for the money!

Some places they will take a photo of your License Plates and send a Bill (coul be a 10 Times the toll) other they will not let you pass, or pay credit card.

the shallow side of the question is in jeopardy but the high side is there are no mistakes in this world.only chances to try and come up with some, you always come up with a good story so now is your chance to explain how you did it. see allot of expressway are set up so you can get back off in a few miles just don't panic and make sure you got plenty of gas, and drive til you get to a exit, there are times when you just have to look cheap in this world and put on a happy face it fun to screw up makes the rest of them feel good. if you stop at the exit and wait and look near the pay booth allot of money is laying on the ground there and you wouldn't be the first to pick it up and move it in to the box

well hopefully they would give you some change to take care of that if not i would ask another driver for some change, or maybe they should have a atm at the tolls

You could buy an EZPass in case that happened in the future. All toll booths have EZPass for those who have that and no cash.

Personally I have found they take credit cards and cash cards even in South Africa !
If you were totally broke my guess is they would let you go, they have an endless stream of customers, and if one fish gets away - so what !

That is why you always have a "stash" of cash somewhere in your car, like people that have "nugget money", my girlfriend has that in her purse for her daughter. If they want nuggets. But yeah I have always wondered that too... hmm what would they do!
