Do jumping jacks, jumping rope, treadmill, etc for a light warm-up before starting. (5 min.)

chest/triceps -- push-ups (vary width of hand position; wide for chest, narrow for triceps.
biceps/back -- pull-ups/chin-ups (wide overhand grip for back, close underhand grip for biceps)
triceps/shoulders -- dips between chairs (vary elbow position, but be careful that your chairs are stable)
core - situps (crunches) for abs and lying hyper-extensions for low-back

I would also add lunges to help increase balance and additional core muscle strength. Lunges will increase blood-flow in your entire body which will help in building muscle.

Do three sets of each with no more than 30 seconds rest between sets and exercises three times a week (example; M-W-F).

When starting do 1 set each exercise for the first two weeks and then increase to 2 sets for the next two weeks and then 3 sets thereafter. Do the last set to failure, but be careful. And as always, check with your Doctor before starting any exercise program.

chest, triceps -- push-ups
biceps, back -- pull-ups/chin-ups
triceps, back -- dips (be careful if using chairs)
wrist, forearms -- gripping exercises

For shoulders? Hmm...

Maybe do push-ups with your feet on a high, raised surface so that your head is inclined towards the floor, putting more emphasis on shoulder muscles when you do the push-up?

Some might say hand-stand push-ups, but that might be a little advanced for your case. Maybe do it with your body against a wall for safety. And have a spotter.
