I was taken on a field trip to a sewage plant some years ago (yes, my uni course was a bit budget!)
They filter out the big stuff with a grid (surprising amount of goldfish in there) and then leave it all in big ponds filled to the brim with bacteria for a few days. The bacteria naturally feed off the nasty stuff and out comes clean water (well mostly). Most places then use UV light it to sterilise it but I don't think its required by law. Its then let into the river.
You'd think it was more high tech than that, wouldn't you?

the water is taken out of the sewage and the solids are put in a huge cesspool that is at the sewage plant where various bacteria consume the organic solids. plasics and stuff (aka. tampon tubes, Q-tip shafts, ect.) are screened out and deposited in a landfill. the water that is removed is filtered and clorinated and sent back into our drinking water system. people freak out when they hear this but we drink the exact same water the dinosaurs crapped in. our planet recycles its own water thru evaporation and rain. no water enters or leaves our atmosphere (in any sizable amounts) so the water is the same that has always been here forever.

It is (almost) biologically neutral after treatment and fairly harmless, the active stuff having been digested by micro-organisms with which the settlement tanks have been "seeded". A lot of it is burnt in special sewage incineration plants, and believe it or not, some is actually used to make plasticine (after sterilisation).

If you live here in Las Vegas it flows out and dump into Lake Mead. Then the water intake pipes picks up the water from Lake Mead and pumps it all back to Las Vegas into water holding tanks throughout the valley. These water holding tanks supplies us with our drinking water. So our sewage just make a round trip to Lake Mead and back into our kitchens and into our drinking glasses of water and back in our bodies where it started from. That's why we buy bottle drinking water.

If I understand correctly, the large organic particles are filtered out, rest are consumed by bacteria up to the point where they chlorinate the water.
