If a diabetic has ketones, their breath will smell like acetone. This is dangerous and requires immediate medical attention. Besides that, no, their breath should not have an odor.

They have a characteristic Sweet or Fruity odor due to the production of a ketone body called Acetone. Ketone bodies (acetone, acetoacetic acid and beta hydroxy butyrate) are produced when glucose is not available to the cells for energy production (during diabetes mellitus and starvation). During DM, all the glucose in the blood remains in the blood and cannot enter into the cells for energy production due to the lack of Insulin. Ketone bodies due to it's excess production in the body leads to it's excretion in the urine which is detected by two tests: Rothera's Test (for acetone) and Gerhardt's Test (for acetoacetic acid).

It is called Ketone acidosis and even non diabetics can have this smell if they don't eat for the day.
Acid will build up and produce this smell.
When us diabetics go on dialysis we will taste this all the time,it is part of the cleaning stuff they use, then, it has nothing to do will the blood sugar levels at all.

if on insulin, yes.
oral meds, not necessarily.
however if drinking (which is a bad idea for Diabetics) they can put off an odor similar to acetone (nail polish remover) in which case they need to seek medical attention ASAP

no not normally if the sugars are out of control then yes they may have a bad odor to their breath but if they are in range of control they should not have a problem the best toothpaste that is recomended for us is colgate total and mouth wash with no alcohol content to it

It depends if their sugars are under control. Also, if they are experiencing an insulin reaction at the time, just had one, or about to have one.
