The real difference between ACEI and ARB is the method of reducing blood pressure. Both medications work with the body's renin-angiotensin system.
ACEI (Valsartan) works by inhibiting the Angiotensin-converting enzyme from converting Angiotensin I (the blood pressure lowering component), to Angiotensin II (a powerful vasoconstrictor <-- it contracts the blood vessels, which causes the high blood pressure by choking the blood flow). ARB (Olmesartan) bypasses the conversion route by preventing the arterial Angiotension Receptor from accepting Angiotensin II.
Discovery of ACEI was accidental, which effects were discovered in snake anti-venom research. ARB was purposely designed to fight vasoconstriction, and has fewer side-effects (so drug compliance is higher).
ARB is newer, and now being prescribed as the drug of choice in fighting high blood pressure (HBP) in the USA. It's use in protecting the kidneys from Chronic Renal Failure (CRF), and it's consequence End-Stage Renal Failure (ESRD), is very important for heart health -- kidneys and the heart are so interlinked that when treating a heart condition, the renal function is also watched carefully.
But with all ACEI and ARB medications if the patient has low-Renin, another Hypertension medication will have to be added with ACEI and ARB to work effectively (ACEI and ARB work poorly for those patients). Which is why it's important to get a tested for Renin levels for effective Hypertension control.
