An oral surgeon, a qualified general dentist (as I've done several), or an ENT surgeon.

My choice would be the oral surgeon, as this is really a routine thing for them.

I would think that an oral surgeon could do it. Possibly a cosmetic surgeon. I know that some do work on cleft palates.
