First, a little meteorology 101. Rain/snow is often associated with an area of low pressure (the big "L" you see on weather maps). These are areas of lower air pressure.

A balloon will pop as it gets higher in the atmosphere because the surrounding pressure decreases as it gains altitude but the pressure inside the balloon remains constant. Well, joints have little balloon-like cushions around them. When the air pressure drops as an area of low pressure approaches...these cushioins swell, causing pain.

Changes in temperature and moisture levels can also cause pain, but this is the most common cause of what many people call "rain-knee."
