Go to the vet. Any sudden change in bathrooming habits can mean the pet is sick. She may have a bladder infection.
If the vet says she is fine, start over house training as if she was a puppy you just brought home. Sometimes dogs need "refresher" courses.
Good Luck

Obviously something has changed - where is she urinating now? You can clean up a spot that she has urinated on with one of those puppy pads, this way she can sniff her own pee and that might show her and reinforce for her that this is where her pee needs to be

You might try a different brand of puppy pads

You also might try one of the new dog litters and litter pans that are now available for small dogs.

Try to think back to what happened right before she stopped using the puppy pads - did anything at all happen after the last time she used the pads and before the first time she urinated somewhere other then the puppy pad? This information may give you some insight as to why she is avoiding the puppy pads now.

Put it in a box, then put her in the box, and wait until she goes, do this about every half an hour to an hour, depending on how much she goes. I agree with the above answerer, also try getting her a new pad, but the box thing should work. It should also help potty train her! Good Luck!
