It could be that you have some teeth rotting in your mouth, but I find that other than that, a really healthy strict diet is the only thing that would help in the long run.
Before I begin, I have to tell you though that this isn't really a suggestion, since I don't think a lot of people would hate their bad breath so much that they'd try this to cure it.
I have followed a strict diet in the past for health benefits and surprisingly noticed some side benefits that I never quite expected; my bad breath was gone, along with smelly feet. By that I mean when I wake up in the morning I don't have that soury yucky smell/taste in my mouth, this continues throughout the day, by that I also mean that I did not have to worry about my feet starting to smell in the middle of the day, they just don't smell at all anymore.
The "strict" diet I had at the time, was basically the 'Fit for Life' diet, with some minor "variations" here and there. That's why I won't recommend it, you should all know that it's hell.

Brush your teeth really good. For some reason doing it while showering makes you do a better job than when youre out of the shower. And brush your teeth really well. Make sure you get the bacteria way in the back. And always floss, cause sometimes the stuff you eat, gets caught between your teeth and that can cause bad breath. Finally, just make sure you always have sugar free mint gum with you.

At least twice a day
brush and floss teeth
brush tongue
use mouthwash
use those breath strips that dissolve on your tongue - use the Listerine kind that kill germs
see a denist
if these things don't help it may be coming from your stomach, go to a health food store and get cholorophyll capsules

All of the suggestions above are really good, but sometimes bad breath can be a sign of ulcers and other medical conditions. My suggestion would be to go to your dentist and find out if it has anything to do with your oral health. If the dentist rules out tooth decay and poor oral hygiene, I would suggest seeing your primary care physician next. Whatever is causing it, it needs to be addressed for your well being.

A lot of people with bad breath have bad tonsils or bad gums or an infection. Have you checked with a Dr?

spew, you're gone, your breath is really strong. wait, come back, i got some Tic-Tac. not 1, but 2, just enough for you.

try brushing your teeth more often, like after you eat. a friend of mine brings a bottle of mouthwash and floss and carries it in her purse.

Brush at least 2x a day, floss daily, use mouthwash twice a day, brush your tongue also when u r brushing your teeth, and chew gum or mints.

Sounds weird, but.....brush your tongue.

Sometimes sinus infection drainage will produce horrendous exhales, too.
