Personally, I Kinda like the "break" of the pressure to come up with the ice breaker lines. If its not mutual you'll know quickly. I was very flattered when my wife came up to me and started the converstation. It was relaxed and enjoyable, But then she is wonderfull and I got lucky.
I dont think women taking the first step is offensive. Truth be told, I've learned to even enjoy when she is sexually aggressive too. Im no sissy man but love "submitting" to her and pleasing her. But we both take turns being pants.

It depends on the guy, I always find the best technique is just to say hi and chat to them, if you're going to the bar ask if they'd like a drink or anything and if you get on and like him just make it clear that you like them but leave it at that, if they say they like you too then go in for it but if they dont then at least you've now got a new friend! It always works for me :-D
