Remove the pillow from your bed and place a towel there instead. Put a few drops of mineral oil into your ear canal, lay on your side for 5 minutes with the affected ear facing up. Turn over and lay with the affected ear facing down, against the towel. Remain this way for 5 more minutes. Repeat the mineral oil and postioning again if needed. If the cotton doesn't work itself out, see your doctor who can safely extract it with forceps. Call ahead and explain your situation. You may be worked in between scheduled appointments. You don't need to go to an ER as long as you don't put off seeing your doctor. But be sure not to wait more than 2 or 3 days as the cotton will accumulate bacteria and may trigger infection. A vacuum cleaner will not "blow out" your ear drum but may be ineffective.

This actually did cause me to LOL, and astonished my dog.
While the sage advice is to either get it out with tweezers, or go to the doc, although I, as well, would be tempted to try the vacuum thing. Do you have a less than full power vacuum? Like one that you vacuum off the keyboard with, or one of those pathetic anemic ones they advertise you can plug into the car cigarette lighter? We tossed the one we bought after one try, is was so weak, but might be just right for an "ear job".
If you don't want to foot the bill for a doctor visit, go to the pharmacy, they have these kits where you clean impacted wax out of the ear by squirting water/hydrogen peroxide mix into it, might just float the thing out.
I also shove the thing right down to my eardrum and twirl away, it is loud, but the only thing that I feel gets my ears clean. No untoward events yet, maybe it is just a matter of time.

Vacum may be a good idea, but if I were you, I would try and GENTLY remove it with tweezers.....I know it is probably not the best way but so long as it is not far in,BUT I have done it before....It is annoying but it would suck to have to go to an expensive doctor for this and they will do the same thing, only safely.....Good luck!

You twit. Get a pair of tweezers, and have some one remove it for you. Do Not use a vacuum. You could seriously damage you ear drum. If it is in to far, you should probably go to the ER to have it removed. And don't do it again.

Have someone hold up a straw next to your ear and suck in. Beats vacuum cleaner... Now if they choke on it and we get another question from you about that, I wouldn't know whether or laugh or to cry. Use a small/narrow straw just in case...

I've actually done this before. Very carefully use a pair of tweezers to grab the cotton and pull it out

Because it could lead to infection or permanently damage your hearing, get to a doctor immediately. They'll get it out in no time. Don't go to the ER, it's not necessary in the case I don't think.

You are going to have to go to the Doctor. They will use the needle nose tweezers to get it out. This happened to me, and I couldn't get it out myself. My mom tried too with tweezers but no luck.
