I am a dual citizen (USA and UK) and I need to get a replacement citizen born abroad birth certificate. What is the name of this birth certificate and where can I get one. I was born in England and I am living in the US.

The form is FS-240, Consular Report of Birth Abroad, and here is how to get a replacement: look about 2/3 of the way down the page.
