You really need to see a dermatologist.
It could be a parasite of some sort, a friend of mine came back with similar symptoms in a different area from her trip to Africa, and if it is spreading I would go as soon as possible.
Also if you are a scuba diver I recommend seeing a diving doctor as it could be a form of DCS that you failed to diagnose at the time.
There's only so much anyone can say not having seen your hands and not knowing your medical history. If your hands are wet a lot then it could be something connected to your work. Either way, you need to see a doctor.

i would have my liver checked just to be sure its not a sign of diseases such as hcv, nash ect....
i too suffered from a rash on the hand for over 5 years. looks like teeny tiny clusters of blisters and they break open and cause the skin to crack. it got so annoying the doctor said i was simply allergic to water...well, thats like being allergic to one's self!
it did not go away no matter what...UNTIL....i found out i had HCV and did antiviral chemotherapy...the rash has not shown face not once and I am post 5 years chemo now. (i also have a svr)
HCV does not show up on standard blood labs. ALT and AST levels can or are sometimes not affected with the natural course of HCV, so an antibody test is in order. Regardless if you have the more popular risk factors or not. 1 in 4 are infected in the US alone and doctors treat the bi-directional diseases HCV causes instead of the HCV itself because they usually just run a standard blood lab.
Take this answer with you on your next visit to the doc. He will understand.

Oh my god.... A parasite? It's eczema ... I have the same thing. I put my hand in cold water to help soothe the soreness. I noticed its triggered by heat. I can't wear closed toe shoes because my heat can't escape from my feet so it all goes to my hands and creates little bubbles under my skin. I would just let it run its course, dry out whatever and make sure to keep your body temperature at a normal comfortable state. That's what helps me at least.

try a hydrocortisone cream from your chemist .I had the same problem but when I used this cream for my kids it cleard up my skin on my hands as well.

sounds like contact dermatitis. see a doctor and wear gloves when your hands are in water.
