The best thing to do is take regular breaks. The other thing that is important is that you should look away from your computer and focus into the distance for a few seconds every ten-fifteen minutes.
It may also be worth checking your eyes with your optician/ophthalmologist, and checking your lighting arrangements.

I had a similar problem where I couldn't focus on objects fast enough. Things would be fuzzy for a like a second before becoming clear. I saw a specialist last month & sh told me that I will be prescribed for a set of glasses. She said that it wouldn't be permanent as the problem is not my eye but rather a muscle inside it which has undergone strain from the computer screen. She said that the glasses would relax the muscle & train it to work properly, that is relax & contract on time so that my focus doesn't have a lag. If I were you, I'd see a doctor soon because it gets worse as time goes by.

Take break from you computer every 3 to 4 hours. If you can't do that then there are glasses that you can get to relieve the strain of the computer on your eyes. I have a pair and they work great. There are some eye drops available over the counter that are suppose to be for computer eyes but I have never tired them.

Eye blinking rate decreases dramatically and leads to dryness in the eyes. So don't forget to blink now and then.
Keep one arm length from the monitor
Use anti reflective glasses.
Try using 'refresh tears' eye drops
Look at different distances now and then. I mean you can look at anything other than monitor. A tree or a book near.
Rest, rest and rest!
