Most people, especially in the US, use the two terms to mean the same thing, and if you buy any of the hot cocoa or hot chocolate mixes available at your supermarket, they’ll pretty much taste similar regardless of how it’s labeled. However, there is a difference between the two.
Technically, hot cocoa is made with cocoa powder, and cocoa powder is actually something that has no fat content which, in this case, is called “cocoa butter.” Most of the time if you even encounter cocoa powder, it’s the stuff that comes in a metal can and is used for baking purposes, and is unsweetened.
Hot chocolate is actually made from melted chocolate, so you’ll have the cocoa butter contributing to the fat content, resulting in a considerably richer beverage than you’d get from what was the original hot cocoa recipe.
There are some commercially available hybrids out there that sometimes package themselves as either “hot cocoa” or “drinking chocolate,” but these will certainly make use of the fat in cocoa butter in one form or another because its use results in a far more appealing beverage for most people.

There's a very slight difference between 'hot cocoa' and
'hot chocolate' in the drinks, but usually them mean the same
and are mentioned in different names.
The slight difference is hot cocoa is made from cocoa beans
and grind into powder form and has no fat content called
'cocoa butter', whereas hot chocolate is melted chocolate mixed
with cocoa butter in the fat content which is alot more creamier
to drink.
