Yes, phone ahead if you can or have someone do it for you & explain your condition. The lorazepam is perfect, hopefully he will give you enough that you can take 1/2 45 mins before the appointment & the other 1/2 15 minutes before. Also request they give you nitrous it can help a lot as well if they have it. If they don't consider switching to someone who does. & don't be afraid to switch if they aren't sympathetic.
Other tips: take headphones with some relaxing music that you like it really helps when you are in the chair a long time. If you have a particularly bad moment try counting backwards. I usually start @ 99 but start @ the 1st # you think of & it will keep you more distracted. I also tap the pads of my fingers to my thumbs (12344321) or if my hands are together tap the finger pads together. It's supposed to be good for youreflexology or something.

there is something called stress reliever, it's all natural and you get it at the health and nutrition stores. This will relax you and won't cause any side effects with the meds they give during and after the visit. I have to have it when I go to the dentist, because of another dentist's bad experience. Believe me there are some really great dentists that make it a whole lot better to go to!

I've had the same problem for years now... I had a traumatizing experince at the dentist's when I was a child and I couldn't go ever since. Recently, I had all my teeth done and porcelain veneers and all that, but under IV sedation.
I guess it depends how strong your fear is.
Here is a site where you can find many tips and stories from people who went through the same things:

1. Back out of a couple of the clubs. Only do your favorite thing(s). 2. The smell of lavender is amazing. Get some lavender lotion or oil and it will calm you down. 3. Make time for yourself. - Take a hot bath at least every other day. 4. Eat healthfully 5. Get at least 7 hours of sleep every night 6. Do NOT use caffeine. This overworks your body and makes you more tired in the long run. 7. Get exercise - even a 15 minute walk a day will do amazing things for stress levels. :) :)

I just think about how much better I will be off after I've visited the dentist. That works for me but I imagine not others.

in my own opinion not going to the dentist will only cause more pain down the road so anything that happens there may be somewhat painful but not nearly as bad if not going
