It is Italian for "I love you always".

It is Italian. Word-for-word, "ti" is "you."

"Amo" is the first-person singular conjugation of the verb "to love," so it means "I love."

"Siempre" is "forever," and is an adverb here, modifying "to love" (it's saying how the speaker will love you, and in this case it's "forever).

So for all that you get, "I love you forever." Very romantic!
