relax and concentrate on your breathing. When your thoughts drift, bring them back to your breath, and realize the only thing there is is the here and now.

Staying present = being in the moment = staying focused on what you are doing.
If you are sitting in a chair, feel your feet touch the floor, the pressure on your back, the breath going in and out of your mouth. If you are washing the dishes, feel the warm of the water on your hands, see the movement of the suds over the dishes. If you are walking, feel your feet as the heel, then the toe strikes the ground, as the other leg picks up and moves forward, the movement of your hands and arms as you walk.
Be aware of what you are doing and everything around you.
As an aside, dissociating - not staying in the present, but just sleep walking, happens as a part of the fight-or-flight response (the "flight" part) when we are stressed out and there is nothing to fight against and nowhere to run to. De-stressing (meditation, breathing, exercise, an easy walk) helps you to be able to focus.
From a nutritional standpoint, dopamine (a brain chemical) helps keep us focused. Especially in the morning, be sure you have a good protein breakfast (proteings encourage dopamine - the wake up chemical; carbs encourage the relaxed/sleep chemicals).

It's called mindfulness.
When you eat, be mindful of the food -- don't read a magazine at the same time.
When you walk, sense all that there is to sense around you -- sights, sounds, smells, the feel of each footfall.
When your mind starts to wander off to other things, gently bring it back to where you are.
