Two games come to mind (I have actually had to play these at parties)
Game #1:
Give everyone paper and pencil. Have each person make a list of everyone at the party in the order they met you!
It's hilarious. Have gag gifts for the person who has known you the longest, came the furthest, etc.
Game #2:
Make up tags in sets of two: Like 'Bogey' and 'Bacall'
or 'Dagwood' and 'Blondie' or 'Mutt' and 'Jeff'. Stick them on your guests' backs at random. They then - without asking anyone, "WHO AM I?" but by only asking questions that can be answered yes and no, arrange themselves into pairs. Then, Bogey with be with Bacall, Dagwood with Blondie, etc.
Happy 45th birthday!

I like playing cards, and they might enjoy games like Pit, Taboo, and Trivial Pursuit, so you could make those available in case anyone's interested. It's not very hawaiian, but you could set up a croquet set. As far as entertainment, keep music playing the whole time. Perhaps hawaiian music for the beginning of the party and/or dinner time, and then whatever kind of music the birthday person likes after that. If it's at night, have tekee (sp?) torches, and perhaps a bon-fire. Everyone likes roasting marshmallows! Have pinapple upside-down cake, too. Okay, I hope you can use some of these ideas! Have fun!

Um...most adult parties I've been to don't have games. Who ever said that standing around talking wasn't fun? Provide some music, a place to dance and plenty of tropical drinks and everyone should have a good time.
