To add more detail my dad has had 2 heart bypasses the first repairing 3 valves back in 1988 and the 2nd in 1997 repairing the previous bypasses plus another 3! And fentynal patches are used along with oral morphine to ease pain he only has very low percentage of veins/arteries working in his heart now. He is classed as terminally ill and people like him which is becoming increasingly common are now being helped by cancer hospices.

There is a new drug for refractory angina - Ranolazine, that might help.

Other options include EECP [Enhanced External Counterpulsation] and TMR [Transmyocardial Revascularization].
TMR is only performed in specialized centers.

Don't have enough information to know if your dad is a candidate
for any of these, but these [and heart transplantation] are the options available for refractory angina.

I've actually never heard of acute refractory angina. Angina is usually a term that refers to chest pain that is brought on by exertion, and is relieved with rest or by taking nitroglycerin. There are several types of angina, but I've never heard of the refractory type. Generally, if angina is not responding to medications prescribed by a doctor, then a stress test is done, or perhaps coronary artery catheterization (to fix blockages in the arteries that supply the heart).

In addition, I've NEVER heard of fentanyl patches being used to treat angina. Based upon the limited information you've given me, I would strongly recommend a second opinion. Good luck.
