The 7% hypertonic saline solution that is used in nebulizer treatments is, and should be, sterile. The nebulizer used to dispense it should be cleaned per its instructions regularly. No doctor would recommend you mix the solution yourself.

If you insist on mixing your own, you need to keep STRICT STERILE TECHNIQUE.

Sterile 20% saline and sterile water for injection.
Draw 1ml of 20% saline and add to 2ml of water for injection in a sterile syringe.

As I recollect, isotonic saline refers to the concentration of salt in the blood. So, to me you can specify 7% saline or isotonic saline. I'm not sure about the nebulizer solution but making a 7% saline solution is easy. Mix 7 grams of sodium chloride (table salt) into 100ml of water. You can convert this to ounces but I always find the metric system so much easier. I don't remember the concentration for isotonic. Perhaps it is 7%.

People with Cystic Fibrosis can pass bacterium like cepacia, pseudomonas, MRSA, etc... to each other. I have a seven year old daughter with CF so it's always a huge concern when we are in hospital.... trying to avoid other CFers. It's really kind of sad as who could understand what a CFer goes through more than another CFer? They used to have ''CF camps'' until they realized they were picking things up off each other. Now in hospital, they are all separated from each other.

Your CF team should give you a prescription for the nebulizer saline or, if your insurance won't cover it/you can't afford it, either the CF team or your respiratory care team at the hospital should be able to give you the appropriate recipe. Be sure to use sterile water when you do make it - tap water is not appropriate.

7% would not be isotonic. I suggest you buy the saline for use.

People with CF all carry germs, all of the time, in their airways-both lung and sinuses. However, not all people carry around the same germs. Some people carry more problematic germs (more likely to cause problems to the person with those germs) than others do. Unfortunately, those germs can definitely be passed from one person with CF to another, possibly causing risk to the newly infected person. So, we ask that people with CF avoid close contact with others with CF, unless they are family members.
