Really need to ask the doctor about this. If the baby has holes in it's heart, the baby should be seen by pediatric cardiologist.

weight is not a factor unless the child was born very prematurely. downs syndrome might be a factor in itself, but in general the two "holes" in the heart which all newborns have, close up naturally within a couple days of birth. beyond that the doctor will assess the child for how well it is oxygenating and generally thriving. some children go into early toddlerhood before having anything done. if these two holes do not close naturally surgery is necessary.

the reason why babies have holes in their heart is because there used to be a blood vessel that's connected to that hole. one blood vessel is called "ductus arteriosis" which is connected from the pulmonary artery to the aorta -- it bypasses the lungs because babies don't need the lungs yet when they're inside the mother's womb. (if you want, i can explain why they can live inside the belly without breathing).
when they come out, the doctor makes them cry so that their lungs will open up and the artery will just go away... this happens immediately as soon as the baby starts breathing. the hole -- takes a couple of days for it to close. some cases, the hole does not close at all. same thing with my cousin -- the hole never closed completely (just partially) she was doing ok, but the only life-threatening condition that happened to her years ago was when she had 'strep throat' which was left untreated. the bacteria traveled through her blood and ended up in her heart -- in that hole... and this caused a lot of complications, including stroke.
the hole may not be life-threatening for now, but that means you have to be extra-careful if so. but this is a fairly rare case -- majority of babies being born, the hole closes up in a couple of days.
