I don't think it's bad manners, but it means you're being evasive and you just don't want to answer the question. For instance, if someone asks you a direct personal question that you consider shouldn't be asked, you can simply reply: 'Why do you ask?' This is an indication to them that they've been ill mannered in asking!

No, it's not bad!
If your a caring & detailed person, you would like to make sure the question is understood. Sometimes, it will require maybe many questions to put the orginal question asked into focus!
A not clear defined question can create problems & misunderstandings - Look at our world today! People die for no reasons! BAD judgements are made from not asking the right questions.

Do you really need to ask a question about questions as answers being bad manners? I don't know about it being bad manners, but it's annoying as hell. :)

Socrates, the philosopher who lived in ancient Athens, used answering questions by questions as a method to teach philosophy. If you do it correctly you can show the person who asked what the answer is by making them think... Sometimes people ask questions because they are too lazy to use their brain!
Of course some factual questions need to be answered properly!

Depends on what the question is, sometimes people don't give enough information. I think it's fine as long as you are not insulting a person who asked a legitimate question.

No, don't we live in a free society these days, you don't have to answer anything you don't want to and should never force your issues or opinions on anyone either, people should be able to make their own minds up.

Not necessarily - if a person ask a really personal question or it's something that I don't feel is appropriate, I always say "Why do you ask? It usually stops that person from being so noisy.

No. Depends if the orginal quesiton was stupid/ignorant/lack of thinking and needed emphasis or if it was a legitimate answer to a hard quesiton.
