Is it a disease or birth deformity? A small child sized head with a normal size adult body.

It's a birth defect. The head doesn't shrink, it's that it doesn't grow properly. The medical term is 'microcephaly', and in the past some people with it were called "pinheads". They are usually, but not always, mentally retarded in some way.

A head with a boney skull in it cannot shrink, although the brain itself can. There is a condition called "microencephaly" which describes an abnormally small head and brain.

The head doesn't shrink in those cases, it just never grows very much.
