If your premium had lapsed before the accident occurred, if there was a fraudulent claim involved, and if it was found through investigation a criminal act occurred through the accident happening due to contributing factors on your part

It can be denied if the accident happened when the car wasn't insured
(expired, etc).

It can be denied if the information submitted to the police or insurance
company was knowingly false.

It can be denied if the insurance company determines that there was
callusion between the parties to defraud the insurance by getting into
an accident.

It can be denied when the accident is caused by various "acts of God"
type events. For instance, if you were in an earth quake and your
car ran off the road, your insurance company might have a clause
saying that it isn't covered.

Lapsed policy, fraud, using the vehicle in the act of a felony, an excluded driver causes loss, damaging vehicle on purpose, an act of war, damage caused while being driven by mechanic, incorrect information given when policy was purchased. Probably a dozen others I can't think of right now.

I once knew of someone who said they wouldn't have any drivers under 25 years old driving their car, and then their son who was 19 was involved in an accident (which was not his fault) and the agent refused to be involved.
