Its all done by computer. Slots are left in the broadcast where local stations insert their commercials. They just may be off by a second.

You ever notice the providers commerical comes on over top of the regular programmed comercial. Like at the end of your providers commericial (ex. Comcast, Dish, Directv) you'll see like a half second clip of the commerical you were suppose to see but didnt. This is just a way so your provider can advertise and they play it over the stations commercials.

MANY STATIONS JUST DONT DO A GOOD JOB CLIPPING AND BEGINNING NEW COMMERCIALS OR TV SHOWS.

some commercials are long so the entire commercial aired only a few times then they are parts of the commercials and have to cut it off for finanical reasons.

Time on TV is controlled by a computer. No manual control is excercised.

The commercial may have run a bit over it's alloted time.

the show that you are watching only has so much time to play and so they have to adsvertise the comericials that they are being paid for. some of the comerccials are to long but have to be shown so they cut some of them short so they can make their timing and meet the number of comercials need to be shown during the show you are watching.
