There are different kinds. Some give time zones around the globe and others are for high and low tide times. Depends on what you want to keep track of. I had one of the latter but it didn't matter because I don't live on the ocean.

On dive watches, it's used to keep track of elapsed time for the dive. The pointer is set to point to the minute hand at the beginning of the dive. The rest of the ring is marked in five-minute increments showing how long the diver has been underwater (most recreational dives are less than an hour).
As for why it moves counterclockwise, my best guess is that if it gets bumped, it's better to think you've been down for longer than you actually have than the other way around. It's always better to err on the side of caution.

if you noticed, it is marked just as the watch is, so you twist it around to mark a starting point so you can easily time something. it was created for divers, so they could see how long they had been down. ~~~

My dad has a pilot's watch like this. It works kind of like a slide rule.
