It is mandatory if she has adultery, and he may divorce her if he wants to, for pretty much any reason. However, an 11th century rabbi made a decree that either she must agree, or he must get 100 rabbis to permit him to remarry. This is universally agreed to. Also, if either has demonstrated that they hate the other, the court can force him to divorce her. Also, if they get a civil divorce, most (Jewish) courts will force him to divorce her religiously.
