Did you use the test past the expiration date? Otherwise, you may have bought a dud - contact the manufacturer to see if you can get a new one for free.

means you better go get another one. Take the test first thing in the morning for best results. Could have been a defective test.

It means there was something wrong. maybe there was a problem with the test or maybe there wasn't enough urine to read it right. take another test and make sure you have enough urin.
