Nope. It could even mean something as simple as an infection, unless your doctor has said otherwise. HPV only increases factors, and can possibly cause things that are signified by an abnormal pap smear. But you can have one without HPV.

HPV has many variation (strains) to the virus. There are HPV strains that cause warts, there are strains that cause cervical cancer, and there are strains that DON't do anything at all.
A papsmear is supposed to test for any of the strains of HPV. BUT the thing with papsmears is that they are ultra sensitive. One big problem with papsmears is that if you take them too close to your period they can come out abnormal. So having an abnormal papsmear often doesn't mean anything and that is why doctors must do follow up tests.
But if you had an abnormal papsmear that is NOT a false positive then YES the abnormality is caused by a strain of HPV...

No, not exactly, it could be a bad smear, infection of some sort, viral, bacteria, or HPV. They should want to do another one, and there are two I know of on the market designed specifically to target HPV.

Are you a smoker? I was and was told unless I quite smoking it was gonna to effect my test result giving me abnormal pap. results due to my smoking. That's was my doctor's office told me!

hpv is genital warts. if you have warts it will show up. if not it wont.... i kinda dont understand your question
