A malt has malt added to milk and ice cream. A milk shake is simply milk and ice cream. As for me, it's malt all the way, although sometimes when you ask for a malt, they forget to put in the malt and give you a milkshake.

i think malt is more chunky and thicker than shake.
like strawberry malt contains sliced strawberries but strawberry shake is just a juice .
