Call the Human Resources department and explain that you need one. They should be able to send one out to you.

Your employer can provide a replacement W_2. If they don't contact the IRS.

if your employer doesn't give you a copy the IRS can, just fill out form 4506-T mail it it, or fax it. and its free. you can find the form at irs.gov
