3 year old cat has back n leg spams. He would shake his leg. And bites his paws n licks his legs. Any answers?

Been to the vet twice and noone can tell me the cause. On both occasions, cat was given antibiotic jabs, painkiller and antibiotics to be taken twice daily. Problem doesn't seem to go away. Cat is overweight.
