just ask him, and try not to mention that it's your birthday, or it'll be kinda like putting him on the spot. You can tell him that later (maybe still before the dance) and he might get you something nice

Send him an invitation card to celebrate your birthday...print in bold 'Venue: School Dance Hall' on the card.

just say that its your birthday soon and you dont have anyone to go out with and you were just wondering if you could escort (or take) me to the dance on ________ night.

Be confident! Just walk up to the dude and say "Would you like to go to the dance with me?" and see what he says, but no matter what he answers, smile, and say "Cool." and then walk away.
Then when you're alone or with your friends and away from him, then you can freak out!
